Winter/Spring Sports Physical Exam @ PLHS Boy's Gym

The cost is $25.00, cash or check, checks need to be made out to Point Loma HS. 

 

The forms can be found at www.pointlomahigh.com , link to Athletics, then to athletic forms and information, download the athletic packet.  Everything must be filled out on the forms except for the doctor's part, the doctor will administer the physical and fill out the appropriate paperwork.
