Dana Middle School Office

Parents of students who do NOT attend one of the following schools:

Cabrillo Elementary
Dewey Elementary
Loma Portal Elementary
Ocean Beach Elementary
Silvergate Elementary
Sunset View Elementary

...may come to the Dana office any time between 9:15 AM and 3:30 PM to complete the enrollment paperwork and register for classes. Parents should bring the following items:

Immunization record
Recent report card
Copy of IEP (Individualized Educational Plan) if applicable
Copy of GATE Certification Letter
Copy of CST Testing Results
Proof of residence (utility bill, mortgate documents, tax receipts, or military housing orders)
Name and complete address of previous school
Name and phone number to call for emergency
Families of Barnard parents planning to attend Dana in the fall will need to register their student during the week of March 16-20.

IMPORTANT: Families of students currently enrolled at Dana or the schools below do NOT need to pre-register their student because the children are automatically enrolled. Paperwork will be sent home to families in the fall.
